All new Face of Grace Clients are expected to fill out a New Client Intake Form and a Consent Form before arriving for their first facial. If you don't, you will be required to fill out the form before beginning your facial with no price adjustment. Please fill out each question. Your skin is a dual sided organ, impacted by diet, lifestyle, stress levels,  and medications all play a part in the skin you see.

Returning clients will be required to fill out a new Consent Form and a Health History Update Form before every visit. Please note that these forms are to make sure that you as the client and myself as your practitioner are both as well informed as possible to give you the best experience and outcome.
